Can someone explain to me with some reference about determination of MDM ( in E and M.) I am getting lot of queries about MDM from the providers ( physicians ). Thank you .

In my mind, MDM is somewhat of a subjective determination, but two things that might help you are the CPT Guidelines where it discusses this in the E and M section (page 10 in AMA 2013 CPT Professional Edition), and also the Clinical Examples in Appendix C.
